Credentialing Practitioner - Forgot Username

Please follow the steps below if you do not remember your username. 

1. Click Forgot Username? on the Credentialing login page (https://app.vergehealth.com/credentialing/practitioner/login).
   
   
   
2. Enter your birthdate (MM/DD/YYYY) and last 4 digits of SSN. Then click Submit.
   
   
   
3. Pop-up message confirms an email has been sent to your email address on file. Click Ok to close the pop-up message.
   NOTE: The email is sent to the primary email address on file.
   
   
   
4. Check your email for the Account Username Request email from Credentialing Support. Your username is listed in the email. Click Log In to your account link within the email to navigate to the credentialing login page. Enter your username and password. Then click Log In.
   NOTE: If you do not remember your password, please follow the instructions on this page.
